In order to provide the best VBS experience for your child, we need to make sure we know as much about them as possible. Our crew leaders will spend time getting to know them, however some things like allergies, medical conditions, and social concerns take longer to learn about. As the parent/guardian of this child you know them best. We are as inclusive as possible with all children.

If your child has any medical concerns or conditions please make sure they are noted below. Examples of things we need to know about are: allergies requiring an EpiPen and notation if the child carries one, seizures, social concerns, things that are noted on an IEP at school. Failure to omit any medical or conditional information will deem Jackson Free Methodist Church not liable if a situation arises that stems from a condition not made know to us.

Medical and Liability Release

I understand that there is always a possibility that my child may sustain physical illness or injury while at VBS. If this occurs, I hereby authorize the VBS staff and representatives to refer my child to a medical treatment center (hospital, etc.). I further acknowledge and understand that I will be responsible for any medical bills that may be incurred on behalf of my son/daughter for physical illness or injury that he/she may sustain during the VBS. I release the sponsoring organization and its representatives from any claims for personal illness or injury that my child may sustain during VBS. I further acknowledge and understand that my child will be responsible for his/her failure to abide by the rules and regulations of the VBS.

Scuba VBS will take kids deep into an amazing undersea adventure where they'll experience the ever-flowing, never-ending love of God. At Scuba VBS, kids will dive into friendship and be immersed in God's Word. Our VBS is designed for kiddos age 4 to those who just completed 4th grade.

All children attending VBS and their siblings, 18 years and younger, are invited to join us for a FREE dinner from 5:15 - 5:45. Parents please plan to remain with your children during dinner. Children can be dropped off for programming in their classroom (Preschool) or the Activity Center (Elementary) starting at 5:45. If you know of someone planning to attend, but just haven't signed up yet, please have them sign up, it will help with our planning for dinner number and to save time so you do not have to register children the night of.
